Each resident receives an individual assessment and care plan with our highly trained teams, who focus on integrating the comfort, dignity and safety of each resident.

Clinical Programs

Falls Management

Revera provides interdisciplinary falls intervention risk management based on clinical best practices. We assess and address risks from footwear to medication, offering falls-prevention education to everyone: residents, families and staff.

Skin and Wound Care

Skin and wound care is a vital part of overall health, and preventing the development of skin issues is a priority for our care teams. Revera offers consistency in managing skin and wound issues, providing the highest-quality products that are supported clinically through evaluation and research.

Continence Care

Revera provides a continence care program based on evidence and informed practices, where each resident’s dignity, comfort and independence is respected and maintained.

Restraints

Revera is committed to creating a "least restraint" environment, where each resident has the right to make informed choices.

Rehab and Restorative Care

We address our residents’ key priorities such as reducing falls, reducing the impact of osteoporosis, decreasing the pain of arthritis, enhancing abilities after a stroke, managing the progression of dementia, providing post-surgical support, and more. Our teams work with residents to develop personal goals and programs.

Dementia Care

Revera offers a number of programs to help residents living with dementia. We take a holistic approach with our Montessori-based Dementia Care Program, which has been shown to increase levels of engagement and participation in people with memory loss.

Our programs look into everything from specially designed environments and tailored clinical service to customized recreation programs.

Our skilled staff brings respect, compassion, understanding, and geriatric expertise to the care of residents, helping to stimulate their abilities and enjoyment of life.
